ClickTime Digital Forms scoring allows you to automatically calculate a score for completed inspections, audits, checklists and other forms.
You can assign scores to answer options, set a minimum passing percentage and identify answers that should automatically fail an inspection.
When a form is submitted, ClickTime calculates the final score and determines whether the result is Passed or Failed.
This feature is particularly useful for workplace inspections, safety checklists, compliance assessments and site audits.
Scoring is configured individually for each form.
To enable scoring:
Open the Forms module in ClickTime.
Create a new form or edit an existing form.
Enable Scoring on This Form.
Enter the Pass Mark (%).
Configure the scores for the answer options in your questions.
Save your changes.
Scoring is disabled by default. If you do not enable it, the form continues to work without calculating inspection scores.
The Pass Mark is the minimum percentage required for a form to pass.
For example, if you set the Pass Mark to 80%:
A score of 90% results in Passed.
A score of 80% results in Passed.
A score of 75% results in Failed.
An inspection can also fail if an answer is configured to trigger an automatic inspection failure, regardless of the final percentage.
Scoring is supported for the following question types:
Yes/No
Dropdown List
Multiple Choice
Checkbox List
Radio Buttons
Each answer option has its own score, allowing you to decide how many points should be awarded when that answer is selected.
Numeric fields do not support scoring. If you need respondents to select a numeric value that contributes to a score, use a Dropdown List or Multiple Choice question instead.
For example, instead of asking the respondent to enter a number from 1 to 5, you could provide these options:
1 – Poor
2 – Below Average
3 – Average
4 – Good
5 – Excellent
You can then assign a score to each option.
The default score is 0. You can change it to any whole number from 0 to 100. Negative scores are not allowed.
Question: Is the emergency exit clear?
| Answer | Score |
|---|---|
| Yes | 10 |
| No | 0 |
| N/A | 0 |
In this example, selecting Yes awards 10 points, while selecting No awards 0 points.
You should configure scores according to the purpose of your form. A zero score does not automatically mean that an answer is incorrect or that the inspection has failed.
For a question that allows only one answer, the maximum possible score is the highest score assigned to its answer options.
For example:
| Answer | Score |
|---|---|
| Excellent | 10 |
| Good | 7 |
| Needs Improvement | 4 |
| Poor | 0 |
The maximum score for this question is 10 points.
Drop down List questions allow respondents to select multiple answers. The scores for all selected options are added together.
Configure Drop down options carefully so that selecting multiple answers produces the intended result. If an option identifies a serious hazard, use the Will Fail Inspection setting where appropriate rather than relying on the score alone.
Not every question needs to contribute to the final score.
For example, you might include questions asking for the site name, inspection date, employee name or general comments.
To exclude a supported question from scoring, set its maximum possible score to 0 by assigning 0 to all its answer options.
The question can still collect responses, but it will not contribute points to the final score.
Some answers should cause an inspection to fail regardless of the overall score.
For these situations, enable Will Fail Inspection next to the relevant answer option.
This setting is separate from the score assigned to the answer.
Question: Is the emergency exit blocked?
| Answer | Score | Will Fail Inspection |
|---|---|---|
| Yes | 0 | Enabled |
| No | 10 | Disabled |
| N/A | 0 | Disabled |
If the respondent selects Yes, the inspection fails automatically, even if the total score is above the Pass Mark.
If the respondent selects No, the answer contributes 10 points and does not trigger an automatic failure.
Important: An answer with a score of 0 does not automatically fail the inspection. The automatic failure occurs only when the selected answer has Will Fail Inspection enabled.
For Checkbox List questions, if multiple answers are selected and at least one selected option has Will Fail Inspection enabled, the inspection fails.
ClickTime calculates the final percentage using the points earned and the maximum possible score.
Formula:
Final Score (%) = (Total Earned Score ÷ Maximum Possible Score) × 100
An inspection contains four scored questions.
| Question | Maximum Score | Earned Score |
|---|---|---|
| Question 1 | 10 | 10 |
| Question 2 | 20 | 15 |
| Question 3 | 5 | 5 |
| Question 4 | 10 | 0 |
| Total | 45 | 30 |
The calculation is:
(30 ÷ 45) × 100 = 66.67%
If the Pass Mark is 80%, the inspection result is Failed because the final score is below the required percentage.
If a form contains no available scoring points, a meaningful percentage cannot be calculated.
ClickTime determines the overall result using two rules:
Passed
The final percentage meets or exceeds the configured Pass Mark, and no selected answer triggers an automatic inspection failure.
Failed
Either of the following conditions applies:
The final percentage is below the configured Pass Mark.
A selected answer has Will Fail Inspection enabled.
An inspection may therefore fail even when its final score is 100%.
When scoring is enabled for a form, the Forms Entries table displays additional scoring information.
This includes:
Pass Mark (%) – the minimum percentage required to pass.
Score (%) – the final calculated percentage.
Pass/Fail Status – the overall inspection result.
These fields help managers identify inspections that passed or require attention without opening each submitted form individually.
The submitted form's PDF report includes scoring information to help managers review the results.
Depending on the configured report, this includes:
The score awarded for each scored question.
The final score percentage.
The overall Passed or Failed result.
Questions where a selected answer triggered an automatic inspection failure.
Questions that are not scored can still appear in the report with their responses. They do not contribute to the final score.
The PDF provides a record of the submitted answers and the inspection outcome.
Yes. Scoring is configured individually for each form, so you can enable it for inspections and audits while leaving it disabled for other forms.
Yes. If any selected answer has Will Fail Inspection enabled, the inspection fails regardless of its percentage.
No. A score of zero simply means that no points were awarded for that answer. The inspection fails automatically only if the selected answer has Will Fail Inspection enabled, or if the overall score is below the Pass Mark.
No. Scores must be between 0 and 100.
No. Numeric fields do not support scoring. Use a Dropdown List or Multiple Choice question if you want respondents to select a value that contributes to the score.
Yes. For Checkbox List questions, the scores of all selected options are added together.
Scoring calculations are disabled for that form. When editing its questions with scoring disabled, the scoring input fields are hidden.
Yes. You can configure the Pass Mark at the form level, up to a maximum of 100%.
